Mr AD who is 67 years with body weight of 120kg. He has an infective exacerbation of COPD requiring treatment. According to NICE guidelines, COPD patients will require 30mg of prednisolone for 5 days to treat an infective exacerbation of COPD. The body weight of a patient can affect the volume of distribution, for example, obese patients will experience delayed maximal effect as prednisolone is lipophilic and stored as fat. In order to reach a therapeutic dose, I will consider in the above scenario prescribing Mr AD a 40mg of prednisolone for 5 days, to treat the exacerbation effectively.
