Q

What does adrenocorticotrophic hormone (ACTH) do?

A

Plays a role in how you’re body responds to stress.
It stimulates your adrenal glands to produce cortisol, which has many functions such as controlling BP, metabolism, blood glucose, and inflammation.

Q

What does follicle-stimulating hormone (FSH) do?

A

FSH stimulates sperm production in males. FSH stimulates the ovaries to produce oestrogen and plays a role in egg development in females. This is known as a gonadotrophic hormone.

Q

What does growth hormone (GH) do?

A

In children, growth hormone stimulates growth. In other words, it helps children grow taller. In adults, growth hormone helps maintain healthy muscles and bones and impacts fat distribution. GH also impacts your metabolism

Q

What does luteinizing hormone (LH) do?

A

LH stimulates ovulation in females and testosterone in males. LH is also known as a gonadotrophic hormone because of the role it plays in controlling the function of the ovaries and testes, known as the gonads.

Q

What does prolactin do?

A

Prolactin stimulates breast milk production (lactation) after giving birth. It can also affect fertility and sexual functions in adults.

Q

What does thyroid stimulating hormone do?

A

TSH stimulates your thyroid to produce thyroid hormones that manage your metabolism, energy levels and your nervous system.

Q

What does an antidiuretic hormone (ADH) do?

A

This hormone regulates the water balance and sodium levels in your body.

Q

What does oxytocin do?

A

Your hypothalamus makes oxytocin, and your pituitary gland stores and releases it. In females, oxytocin helps labor to progress during childbirth by sending signals to their uterus to contract. It also causes breast milk to flow and influences the bonding between parent and baby. In males, oxytocin plays a role in moving sperm.

Q

What is hypopituitarism?

A

Hypopituitarism happens when the pituitary gland is not active enough. It does not make enough hormones. Hypopituitarism can directly affect the pituitary gland. Or it can indirectly affect the gland through changes in the hypothalamus.

Q

What are the main causes of hypopituitarism?

A
  • Head injuries
    • Brain surgery
    • Radiation treatment to the head or neck
    • Lack of blood flow to the brain or pituitary gland (stroke) or bleeding (hemorrhage) into the brain or pituitary gland
    • Certain medications, such as narcotics, high-dose corticosteroids or certain cancer drugs called checkpoint inhibitors
    • Inflammation of the pituitary gland caused by an abnormal immune system response (hypophysitis)
Infections of the brain, such as meningitis, or infections that can spread to the brain, such as tuberculosis or syphilis

Q

What are the symptoms of a GH deficiency?

A

In children, GH deficiency may cause growth problems and short stature.
Most adults who have GH deficiency don’t have any symptoms, but for some adults it can cause:
* Fatigue
* Muscle weakness
* Changes in body fat composition
* Lack of ambition
Social isolation

Q

What are the symptoms of LH deficiency in men and woman?

A

Affects the production of egg and sperm
Women and men may experience a lower sex drive, infertility or fatigue. In children and adolescents, delayed puberty is usually the only symptom.
Women may also have symptoms such as:
○ Hot flashes
○ Irregular or no periods
○ Loss of pubic hair
○ An inability to produce milk for breast-feeding

Men may also have symptoms such as:
§ Erectile dysfunction
§ Decreased facial or body hair
§ Mood changes

Q

What are the symptoms of an ADH deficiency?

A

This hormone, which is also called vasopressin, helps your body balance its fluid levels. An ADH deficiency can cause a disorder called diabetes insipidus, which can cause:

◊ Excessive urination
◊ Extreme thirst
◊ Electrolyte imbalances

17
Q

What are the symptoms of a prolactin deficiency?

A

Prolactin is the hormone that tells the body when to start making breast milk. Low levels of prolactin can cause women to have problems making milk for breastfeeding.

18
Q

What are the management options in hypopituitarism?

A

Hormone replacement therapies dependent on which hormone is lacking

19
Q

How does a prolactinoma occur?

A

A prolactinoma occurs when some of the cells in the pituitary gland (the ones producing prolactin) multiply more than usual to form a small growth (tumour) in the pituitary gland. The prolactinoma makes too much prolactin and this can cause symptoms.

20
Q

What are the symptoms of prolactinoma in women and men?

A

Irregular or no periods
Reduced fertility
Reduced sex drive
Galactorrhea
Increased hair growth on face and body

Reduced fertility
Erectile dysfunction
Reduced sex drive
Breast enlargement
Galactorrhea

21
Q

What are the symptoms when a prolactinoma is large and can compress on the brain or nearby nerves?

A

headaches, and eye symptoms such as tunnel vision.

22
Q

What are the cases of prolactinomas?

A

For most people the cause is unknown. In rare cases, there may be an inherited cause and this is often linked to multiple endocrine neoplasia type 1.

23
Q

How are prolactinomas diagnosed?

A

Blood test to test for high prolactin levels (women checked for pregnancy first)
Imaging test (MRI preferred) or CT
May conduct other tests to find out if the tumor is affecting other hormones (thyroid, kidney function)

24
Q

What are the treatment options for prolactinoma?

A

Dopamine agonists - control prolactin levels and shrink the tumor very effectively
· Cabergoline (preferred, more effective, and less side effects)
Bromocriptine

Also transsphenoidal surgery and transcranial surgery can be an option in some cases
