Physiology Of Lactation Flashcards
W=how is milk made
Lactogenesis
Milk is synthesises in the acini cells under influence of prolactin
Milk is synthesised from glucose and amino acids in the blood stream
Milk is secreted from the acini els across the cell membrane into the lumin of the alveoli
Initiation of lactation
Prolactin is released from the anterior pituitary gland
Prolactin is produced in pregnancy but milk production is suppressed by oestrogen and progesterone
Once place a is birthed and oestrogen and progesterone drop, prolactin activates milk production in the acini cells
Prolactin levels are higher at night time
Prolactin is also released whe the baby is suckled
Prolactin receptor theory
Prolactin receptor sites is open when placenta delivered
Prolactin surges - primes the sites to begin mil production
Receptor sites start to close if not primed
Skin to skin contact and lots of fees inn early days increase potential for long term milk production
What does oxytocin do
Works on myo epithelial cells to expel milk
Milk ejection reflex
Tactile stimulation of the breast stimulates release of oxytocin from the posterior pituitary
Oxytocin stimulates contraction of the myocarditis epithelial cells surrounding the alveoli
Milk is forced into the lactiferous ducts
What does FIL do
Build up of FIL -blocks milk. Production
removing FIL allows milk production
Ho to enable lactation and breastfeeding
Keep mother and baby together
Help others to recognise feeding cues
Allows unrestricted feeding cues
Support mothers to breastfeed a night
Uses dummies with caution
Create safe place - dark cosy
