  1. Alert and aware x3 (awake and attentive, cooperative, lethargic, stuporous, in a coma)
  2. position, comfort or distress
  3. notable odor, excessive thinness or wasting, pale, diaphorectic, wincing in pain, (habitus), way they move
  4. body type, proportions, deformities or asymmetries, nutritional state, tremors, difficulty breathing,
  5. Older or younger than stated age
  6. Clothing (street clothes vs hospital gown), hygiene
  7. Air (room air vs. oxygen vs. ventilator)
  8. Any IVs

  3. Rhythm and regularity (regular, regularly irregular (heart block), irregularly irregular (arrhythmia))
  4. Right/left asymmetry

tachycardia in adults = >100
bradycardia in adults = <60

radial pulse, 2nd and 3rd fingers over both radial arteries
