- Alert and aware x3 (awake and attentive, cooperative, lethargic, stuporous, in a coma)
- position, comfort or distress
- notable odor, excessive thinness or wasting, pale, diaphorectic, wincing in pain, (habitus), way they move
- body type, proportions, deformities or asymmetries, nutritional state, tremors, difficulty breathing,
- Older or younger than stated age
- Clothing (street clothes vs hospital gown), hygiene
- Air (room air vs. oxygen vs. ventilator)
- Any IVs

- Rate (in bpm)
- Quality (bounding, strong, weak, thready)
- Rhythm and regularity (regular, regularly irregular (heart block), irregularly irregular (arrhythmia))
- Right/left asymmetry
tachycardia in adults = >100
bradycardia in adults = <60
radial pulse, 2nd and 3rd fingers over both radial arteries
