phosphodiesteras inhibitor Flashcards
1
Q
phosphodiesterase inhibitor:
- name:
- what are they?
- action:
- what is PDE III
- action (continued)
A
- amrinone, milrinone (primacor)
- non catecholamine, nonglycoside compounds
- competitive inhibitory action on phosphodiesterase (PDE)III
- Inhibition of PDE III decreases hydrolysis of cAMP causing increased intracellular concentrations in the myocardium and vascular smooth muscle.(PDE III breaks down camp)
- increases Ca++ movement into cell, potentiates the effects of catecholamines which increases cAMP causing inotropic effect with vascular and airway smooth muscle relaxation.

Q
phosphodiesterase inhibitors:
adverse effects:
A
- hypotension
- arrhythmias
- thrombocytopenia
- hepatic dysfunction with chronic use
- hypokalemia
- angina
3
Q
PDE III inhibitor:
implications:
A
- hypotension d/t vasodilation (caution with hypovolemic patients)
- may increase outflow obstruction in pulmonic or aortic valve disease
- may increase ventricular response (may need to pretreat with dig if in a-fib/flutter
