PCN 125 BID until 3 yrs
PCN 250 BID until 5 yo
Erythormycin: 20 mg/kg BID

10-15 mg/kg/d
CrCl <60 then 7.5 mg/kg/d
Titration: increase by 5mg/kg every 6-12 wks up to 35 mg/kg
