Right Medication
a. compare MAR (medicine administration record) with original record.
b. check THREE times// before you pull it, before you prepare it, and before you administer it.
Right Dosage
a. calculations & abbreviations.
b. have someone else verify/call prescriber (if needed).
c. make sure measuring device is calibrated// have measurements on it.
Right Client
a. check armband
b. verbally ask pt NAME & DATE OF BIRTH
c. Check against MAR
Right Route
a. route should be stated on med order.
b. verify if med can or cannot be crushed (controlled release, delayed release, and long lasting).
Right Time
a. correct time of day AND interval.
b. know institutional policies (most common is administer as early as 30 minutes before or late as 30 minutes after stated time).
c. medication orders should include frequency.
Right Documentation
a. chart as soon as medication is given, NOT before.
b. document refusals
Right Reason
a. let patient know why medication is being given.
b. know patient’s history.
c. confirm rationale for the ordered medication.
