NSAIDs mechanism
selectively target COX-2 - an enzyme responsible for inflammation and pain
DMARD mechanism
slow acting - weeks to months
pure anti-inflammatory, no direct analgesic
reduce joint damage
most need regular monitoring
DMARD indications
aim to start with inflammatory arthritis on DMARD within 3 months of symptom onset
which DMARDs are not safe in pregnancy?
methotrexate and leflunomide

folate antagonist so need to take folic acid supplements

TETRATOGENIC - must be stopped 3 months before conception in males and females
SE of methotrexate
leucopenia, thrombocytopenia, hepatitis, cirrhosis, pneumonitis, rash, MOUTH ULCERS, NAUSEA, diarrhoea
need to monitor FBC and LFTs
