Pharmacology Flashcards
Onset of action of antacids?
minutes
Antacids ADR?
metabolic alkalosis
MoA of Cimetidine?
- competitive antagonist of H2 receptors on parietal cells
2. inhibits production of cAMP
Cimetidine ADR?
- blocks androgen receptor causing male impotency and gynecomastia
- headache
- inhibits Cyt P-450
- galactorrhea (women)
Cimetidine problems with increasing dose with long term use?
causes up regulation of H2 receptors and enhanced block of androgen receptors
PPI MoA (-prazoles)?
Irreversible inhibit H/K ATPase on parietal cells
-most effective reducers of HCl
Uses for PPI?
- ZE syndrome
- ulcers
- GERD
PPI ADR?
- Hypergastrin
- ECL and colonic hyperplasia
Best use for H. pylori, PPI or H2?
PPI
Sucralfate (Carafate) MoA?
Interacts with HCl to form viscous paste that binds to proteins in ulcers to promote direct healing (belly jelly)
Sucralfate (carafate) drug interactions?
Do not administer with antacids, H2 blocker, PPI b/c it may bind to them
-administer 2 hours prior to sucralfate
Misoprostol MoA?
PGE1 analog that increases mucous
Bismuth use?
bactericidal to H. pylori
Triple therapy for H. pylori?
- PPI
- Clarithromycin
- Amoxicillin or Metronidazole
Treatment of GERD?
PPI
