Adenosine
SVT
1st: 0.1mg/kg IV/IO rapid push (max 6mg)
2nd: 0.2mg/kg IV/IO rapid push (max 12mg)
Albuterol
Asthma, anaphylaxis (bronchospasm), hyperkalemia MDI: 4-8 puffs via inh q20min PRN w/ spacer
NEB: 2.5mg/dose (wt <20kg) or 5mg/dose (>20kg) via inh q20min PRN
CONT NEB: 0.5mg/kg/hr via inh (max 20mg/h)
Amiodarone
SVT/VT (with pulse)
5mg/kg IV/IO load over 20-60min (max 300mg), repeat to daily max of 15mg/kg (2.2g in adolescents)
Amiodarone
Pulseless Arrest (i.e. VF/pulseless VT) 5mg/kg IV/IO Bolus (max 300mg) repeat to daily max 15mg/kg (2.2g in adolescents)
Atropine Sulfate
Bradycardia (Symptomatic)
- 0.2mg/kg IV/IO (max single dose 0.5mg) may repeat dose once in 3-5min, max total dose C-1mg A-3mg
- 0.04-0.06mg ET

Toxins/ Overdose (organophosphate, carbamates)
- <12y: 0.05mg/kg IV/IO initially; then repeated and doubling the dose every 5 min until muscarinic symptoms reverse
- > 12y: 1mg IV/IO initially; then repeated and doubling the dose every 5 min until muscarinic symptoms reverse
Calcium Chloride 10%
Hypocalcemia, hyperkalemia, hypermagnesemia, calcium channel blocker OD
-20mg/kg (O.2ml/kg) IV/IO SLOW push during arrest, repeat PRN
Calcium Gluconate
Hypocalcemia, hyperkalemia, hypermagnesemia, calcium channel blocker OD
-60mg/kg (O.6ml/kg) IV/IO SLOW push during arrest, repeat PRN
Dexamethasone
Croup
-0.6mg/kg per min IV/IO infusion; titrate to desired efffect
Dextrose
Hypoglycemia
-0.5 to 1g/kg IV/IO (D25W 2-4ml/kg; D10W5-10ml/kg)
Dobutamine
Heart Failure, cardiogenic shock
-2-20mcg/kg/min IV/IO infusion; titrate to desired effect
Dopamine
Cardiogenic Shock, distributive shock
-2-20mcg/kg/min IV/IO infusion; titrate to desired effect
