Propofol/Diprivan
Dose: 1-3 mg/kg (1 mg/kg)
Classification: IV Anesthetic/Sedative/Hypnotic. Anticonvulsant, amnestic properties as well as some antipruritic/antiemetic effects.
MoA: inhibits (hyperpolarizes) post synaptic neuron by agonizing GABA receptors, increasing Cl channel opening time = sedative/hypnotic effect.
SE: dose dependent CV/Respiratory suppression, vasodilation, myocardial depression. severe hypotension, bradycardia, pain on injection, Propofol infusion syndrome (prolonged use). anaphylaxis.
Etomidate/Amidate
Dose: 0.3-0.6 mg/kg
Classification: GABA mimetic
Lowers ICP, CRMO2, has the LEAST myocardial effects of all induction agents.
MoA: binds to GABA receptors and enhances the receptors affinity for GABA neurotransmitter.
SE: contributes to PONV, adrenal suppression, ventilatory depression/apnea, myoclonus (muscle twitching), water soluble at acidic pH and lipid soluble at normal pH
Lidocaine/Xylocaine
Dose: 1-1.5 mg/kg IV
Classification: Class 1B anti-arrhythmic agent, amide local anesthetic
MoA: decreases permeability of Na+ in nerve membranes (inhibits conduction) blocking fast Na+ channels inhibiting pain. decreases ventricular arrhythmias and increases fibrillation threshold by reducing velocity of electrical activity of the heart.
SE: CNS depression, seizures/drowsiness, unconsciousness, apprehension, vomiting, bradycardia, hypotension.
Bupivacaine/Marcaine
Dose: 0.25-0.5% infiltration
Classification: Amide local anesthetic, long acting.
MoA: binds to intracellular portion of Na+ channels and blocks Na+ influx into nerve cells, preventing depolarization.
SE: watch for cardiotoxicity with accidental IV administration
Ropivacaine/Naropin
Dose: 0.2-0.5% (max 250 mg)
Classification: Amide type Local anesthetic, long acting (not as long acting as bupivacaine).
MoA: binds the intracellular portion of the Na+ channels and blocks sodium influx into nerve cells, preventing depolarization.
SE: dizziness, tinnitus, tremors, convulsions, hypotension, Cardiac arrest.
Fentanyl/Sublimaze
Dose: 1-2 mcg/kg per dose initially
Classification: Opioid analgesic
MoA: agonist on the Mu Opioid receptors. crosses blood brain barrier. NO histamine release, x100 more potent than morphine. quick onset.
SE: N/V/D, confusion, weakness, constipation, urinary retention, hallucinations.
