Pharm: Pulm II Flashcards
Types of drugs for bronchodilation of airways
- bronchodilators (sympathomimetic drugs)
- Methylxanthine drugs
- Muscarinic antagonist (Anticholinergics)
Two types of sympathomimetic drugs
non-selective & selective
Types of drugs for inflammation/thickening of airways
- Corticosteroids
- Cromolyn & Nedocromil
- Leukotriene Pathway Inhibitors
- Monoclonal antibodies
Beta-adrenergic agonists
-SABAs & LABAs
Anticholinergics (antimuscarinic) agents
SAMAs & LAMAs
What do alpha & beta agonist drugs mimic?
epinephrine or norepinephrine
What tissue do B2 drugs attack?
resp, uterine, & vascular smooth muscle
What is the actions of B2 drugs?
promotes smooth muscle relaxation
Name the short-acting selective beta-2 agonist
- albuterol & terbutaline
Name the long-acting selective beta-2 agonist
salmeterol
SE of selective B2 agonist
- tachycardia
- hypokalemia
- hypoxemia
- dysrhythmias
- tremor
- restlessness
When it says nonindicated in asthma this means…
contraindicated in monotherapy can be used in combo therapy
What is the name of the OTC direct agonist used for asthma?
Primatene MIST (straight epi)
NOTES
anticholinergics + SABA is more beneficial in tx of severe asthmatic attacks
MOA for selective B2 agonist?
selective stimulates B2-adrenergic receptors, relaxing airway smooth muscle
Uses for Albuterol (Proventil)
- asthma
- exercise-induced asthma
Interaction characteristics for Albuterol (Proventil) & Levalbuterol (Xopenex)
- HTN
- hypokalemia
Age to use Neb form for Albuterol (Proventil)
< 2yo
Age to uses MDI form of Albuterol (Proventil)
4 yo or older
Serious reactions of Albuterol (Proventil) & Levalbuterol (Xopenex)
- heart stuff
- hyper/hypotension
- K+ & Glucose problems
Uses for Levalbuterol (Xopenex)
- asthma
- exercise-induced asthma
Uses for Salmeterol (Serevent) & Formoterol (Foradil)
- asthma maintenance
- exercise-induced asthma
- COPD maintenance
MOA for Salmeterol (Serevent) & Formoterol (Foradil)
selectively stimulated beta 2 adrenergic receptors, relaxing airways smooth muscle
BB warning for Salmeterol (Serevent) & Formoterol (Foradil)
- asthma: if monotherapy–> incr risk of asthma-related death
- peds/kids: if monotherapy–> may incr risk of hospitalization
What is the anti-cholinergic (muscarinic) antagonist that is short acting?
Ipratropium (Atrovent)
What is the anti-cholinergic (muscarinic) antagonist that is long acting?
Tiotropium (Spiriva)
MOA for anti-cholinergic (muscarinic) antagonist
reverse vagally (blocks Ach) mediated bronchospasm. May decr mucous gland hypersecretion
Are anti-cholinergic (muscarinic) antagonist typically not used in___?
allergen or exercise-induced bronchospasm
Uses for Ipratropium (Atrovent)
- COPD maintenance
- Asthma exacerbation mod-severe
Uses for Tiotropium (Spiriva)
COPD maintenance
SEs for Anti-cholinergic (muscarinic) antagonist
Caution w/
- glaucoma angle-closure
- prostatic hypertrophy
- bladder neck obstruction
Important methylxanthines
- Theophylline (tea)
- Theobromine (cocoa)
- Caffeine (coffee)
Sustained-release theophylline can improve what SE of this medication?
nocturnal symptoms
SEs for Theophylline
- N/V
- HA
- GI discomfort
- Diuresis
- Cardiac arrhythmias
- Seizures
Uses for Theophylline (Elixophyllin)
- asthma/COPD maintenance
Is Theophylline (Elixophyllin) used in asthma exacerbations
NO
MOA for Theophylline (Elixophyllin)
phosphodiesterase inhibitor & bronchodilator; increased cAMP
What are the 1st line agents for all patients w/ persistent asthma?
Inhaled corticosteroids

- Cough
System SEs of inhaled corticosteroids
- adrenal suppression
- growth suppression
- bruising
- osteoporosis
- cataracts/glaucoma
- psychiatric disturbances
Why do we see systemic effects when using inhaled corticosteroids?
with inhaled corticosteroids, most of it is swallowed. Only 10-20 percent are inhaled
Patient Education for inhaled corticosteroids
rinse mouth w/ water or brush teeth after taking meds
Inhaled corticosteroids (2)
- Fluticasone [Flovent]
- Budesonide inhalation [Pulmicort]
Uses for Fluticasone inhalation [Flovent]
- prevent asthma attacks
- COPD?
Age range for Fluticasone inhalation [Flovent]
> /= 4yo
Can fluticasone be used as a rescue med for asthma attacks?
NO
Can condition can long term steroid use cause?
osteoporosis
