Paroxysmal SVT med
Adenosine
Adenosine dose
Remember, its for SVT
6mg IV, 1-2 min, 12mgIV, 1-2min, 12mg IV
Contraindications/AE of adenosine
Not for a-fib, flutter, or VTach
Common AE: chest pain, HA, flushing, nausea
Serious: card conduction abnormal, HOTN
Symptomatic bradycardia
Atropine
Atropine dose
Remember, its for brady
0.5mg q3-5min max 3mg or 0.04mg/kg
Atropine contraindications
Not for asystole or PEA
AE: tachyarrythmia, xerostoma, blurred vision, photophobia
First line med for pulsless VT /VF
Amiodarone - ACLS 1st line
Other than VT, what else does amiodarone treat?
SVT - rate control/cardioversion and maintenance
Ventricular arrhythmia
Afib (not first line)
Pulseless VT/VF
Amiodarone contraindications
A fib w/ WPW
Pregnancy
AE include: brady, torsades, HOTN, paresthesias, pulm tox and many more
Also lots of drug interactions
Life threatening venricular and supraventricular arrhythmias (Not SVT)
Procainamide
Procainamide contraindications
Not for SVT (toxicity risk)
Prolonged QT (this is a SE of procainamide)
SBP <100, HR <60
Procainamide dose
Regular, wide complex tachycardia:
20-50mg/min
or
100mg q 5min
To effect, HOTN/brady, or QT prolongation
Procainamide maintenance infusion dose
1-4mg/min
Torsades de pointes or polymorphic VT w prolonged QTi
Magnesium
Magnesium dose (for cardiac)
1-2g IV in NS or D5W in a rapid bolus
