Bacterial Infection 3 stages -Catarrhal 1-2 weeks -Paroxysmal 2-4 weeks -Convalescent 2-4 weeks
Consider in coughs >2 weeks

PCR, Reportable to NYS
Pertussis Treatment
Zithromax, erythromycin
Pertussis Prevention
DTAP, TDAP ADACEL
Tuberculosis
AKA white plague Three stages Primary/initial infection Latent/dormant infection Recrudescent HIV patients have a tenfold risk of contracting TB
TB Risk Factors
Homeless, close contact with infected person, immigration <5 years (asian, latin american, african) institutionalized, health care workers, immunocompromised, chronic disease.
TB Signs and Symptoms
Cough, night sweats, anemia, fever hemoptysis, weight loss, pneumonia, bronchitis, pulmonary fungal infection, tumor
Screening every 12 months for at risk individuals
Annual PPD
TB Diagnosis
Positive PPD, Quantiferon gold, CXR, AFB, sputum culture
TB Treatment
NYS reportable, refer to TB clinic. Medications Isoniazid (INH), Rifampin (RIF), Pyrazinamide (PZA), Streptomycin (SM), ethamutol (EMB)
Active disease- multi drug may be needed
HIV- Needs treatment with >3 drugs
Severe disease- 4 drugs for the first 2-3 months.
