When a UMOS is AWOL for 2 Hours at the beginning of their scheduled tour of duty
Desk Officer
Make telephone inquiries including but not limited to:
- Command Roll Call
- Court Sign in Room
- Sick Desk Supervisor
- Member’s residence
Check Department records maintained at the command, including but not limited to:
- Telephone Record
- Roll Call Change Sheets
Make inquiries of command personnel as deemed necessary.
Request that the Desk Officer of the member’s residence precinct have a SUPERVISOR respond to the member’s residence.
If member resides outside of NYC, request the Intercity Correspondence Unit, to make a notification to the local PD concerned and request that they respond to the member’s residence.
AWOL After 4 Hours
Desk Officer will Notify
- Commanding Officer/Duty Captain
- Borough Commander/Counterpart
- Operations Unit
- Internal Affairs Bureau
Make a command log entry of facts and notifications made.
Employment Discrimination
Any NON SUPERVISORY MOS is strongly encouraged to report the complaint or the problem to the EEOD, Supervisor or Manager at any level, or an EEO Liaison
Who must report an EEO?
Any Supervisor, manager, or EEO Liaison MUST report the complaint or problem to the EEOD ASAP OR by NEXT BUSINESS DAY.

- Sexual Harassment
- Retaliation related to EEO

After business hours and on Weekends, the EEOD received messages via office Voicemail and Email and will make RETURN phone calls on the NEXT BUSINESS DAY.
NON-Supervisory MOS may TELEPHONE the EEOD ANONYMOUSLY to file a complaint and/or discuss matters, regarding employment discrimination and/or related retaliation.
NON-Supervisory MOS may report an allegation of employment discrimination and/or related retaliation IN WRITING ANONYMOUSLY by preparing a detailed written report to the Deputy Commissioner, Equity and Inclusion Attn:EEOD

What are the Hours for EEOD?
0800HRS-1800HRS
Monday - Friday
Supervisors, Managers, and EEO Liaisons will telephone the facts during business hours or the next business day
Upon receiving an EEOD CASE NUMBER from an investigator at EEOD
Supervisor/ Commanding Officer/Manager
-Prepare a confidential report on a Typed Letterhead within 5 BUSINESS DAYS to EEOD when a case number is issued. (Not inquiry number)

For Display of Offensive Material
Telephone the facts to EEOD during business hours Mon-Fri 0800-1800 HRS and BE GUIDED BY THEIR INSTRUCTIONS.
After business hours and on weekends leave a voicemail mail and an investigator will contact you on the following business day.
After you leave a voicemail
- Photograph Offensive Material AND
- Take immediate steps to secure, remove, and/or cover offensive material FROM PUBLIC DISPLAY.
- Safeguard the offensive material and prepare a Property Clerk Invoice….Invoice as INVESTIGATORY
- Make a command log entry of details but do not describe the facts or identify the offensive material/object that was discovered.
- Notify Commanding Officer/Manager
- Forward a confidential report on a Typed Letterhead to the Commanding Officer, EEOD direct within 5 BUSINESS DAYS.
- Attach photo of offensive material and a COPY of the Property Clerk Invoice, if applicable
- Include the EEO case number
Off Duty Employment outside NYC involving Firearms
Deliver a copy of the Class 1 Federal Firearms License along with the Application to the Commanding Officer along with the Off Duty Employment Application/Notification.
After subsequent review by ATF deliver a copy of the license to Commanding Officer AND Employee Resource Section
Also provide “Application for License as a Gunsmith or Dealer (Individual) or “Application for License as a Gunsmith or Dealer (Partnership)
When do you Submit a new Off Duty Employment Application Form
1- Report any change of status such as change of Employer, duties, location of employment etc
2- Application must be submitted at least 10 days prior for any continuing off duty employment beyond a 12 month period
Temporary Removal of Firearms for Non Disciplinary Cases
Desk Officer, Command of Surrender
Notify the permanent members command and request a check of the members Force Record to determine if all weapons listed have been surrendered.
Prepare a Property Clerk Invoice for firearms obtained and make a notation that “Property of Uniformed Member of Service-Do not return without approval of the Commanding Officer, Medical Division”.
Place Identification Card in Plastic Security Envelope and seal- DO NOT PREPARE A PROPERTY CLERK INVOICE
Secure firearms and Identification Card in the commands Property Locker
- Make a Command Log Entry
- Do not prepare a Restoration/Removal of Firearms
UMOS remains in possession of their shield.
When returning Firearm to MOS when there’s is no basis for Psychological Evaluation and no basis to remove firearms
Desk Officer Command of Surrender/Members Permanent Command
-Make return call to the Medical Division to verify authenticity of the notification THEN make an entry in the
TELEPHONE RECORD
- Notify Commanding Officer and Roll Call Personnel
- Return the Firearms and Identification Card to the member.
- Ensure the member reports to the Employee ASSISTANCE UNIT on the next business day.
- Make a command log entry

- Crime Prevention Coordinator

What are the four reasons to Appeal an Evaluation?
- Factual Error
- Rater’s interpretation of instructions
- Bias or prejudice on the part of the rated
- Performance Evaluation is COMPLETED and is based on factors other than performance
-Rather will inform the Commanding Officer if the ratee wants to appeal
