peri-implantitis Flashcards
What does peri-implant health require?
Absence of clinical signs of inflammation
Absence of bleeding or suppuration on gentle probing
No increase in pocket depth compared to previous examinations
Absence of bone loss beyond crestal bone level changes resulting from initial bone remodelling
Can have a single bleeding spot around the implant
What is peri implant mucositis?
An inflammatory lesion of the peri-implant mucosa in the absence of continuing marginal bone loss
Bleeding on gentle probing
Other signs of inflammation may be present eg - erythema, swelling, suppuration and increased probing depth
Treatable
No bone loss
What is peri implant mucositis?
Presence of bleeding and/or suppuration on gentle probing with or without increased pocket depth compared to previous examination
Absence of bone loss beyond crestal bone level changes resulting from initial bone remodelling
What is peri-implantitis?
A peri-implant biofilm associated pathological condition, occurring in tissues around dental implants, characterised by inflammation in the peri-implant mucosa and subsequent progressive loss of supporting bone
How does peri-implantitis present clinically?
Presence of bleeding and/or suppuration on gentle probing
Increased pocket depth compared to previous examinations
Presence of bone loss beyond crestal bone level changes resulting from initial bone remodelling
In the absence of previous data:
- presence of bleeding and/or suppuration on gentle probing
- pocket depths of ≥6mm
- bone levels ≥3mm apical to the most coronal portion of the intraosseous part of the implant
What is the prevalence of peri-implant disease?
43% of patients with implants have peri-implant mucositis
22% of patients with implants have peri-implantitis
What are the risk factors for peri-implant disease?
History of severe periodontitis
Poor plaque control
No regular supportive peri-implant care
Smoking
Diabetes
Give 6 local risk factors for peri-implant disease
Submucosal cement
Positioning of implants limiting access to OH
Absence of peri-implant keratinised mucosa
Occlusal overload
Bone compression necrosis
Biocorrosion
Presence of Ti particles within peri-implant tissue
What is the role of a GDP in implants?
Regular routine holistic care
Patient prep - clinical and emotional
Info resource
Making the referral
After care and maintenance
Give 4 reasons that someone may get an implant
Congenitally missing teeth
Tumour resection
Trauma
Endodontic failure
Periodontally related tooth loss
What is primordial prevention?
Prevention of risk factor development (getting everything right before placing the implant)
What is primary prevention?
Prevention of disease onset and risk factor control
What is secondary prevention?
Prevention of disease recurrance
What is tertiary prevention?
Prevention of disease complications
