Inability to register and integrate stimuli and perceptions from one side of body. Draw a clock.

Right/left Discrimination

Inability to identify the right and left sides of one’s own body. Tell pt to point to body part upon command - “right leg” or “left ear,” etc.

Anosognosia

Denial, neglect, and lack of awareness of presence or severity of paralysis. Talk to pt - they will deny or say there is no concern. Will make excuses.

Figure Ground Discrimination

Inability to visually distinguish a figure from the background. Have pt pick objects out from an array of others.

Form Constancy

Inability to perceive subtle differences in forms and shapes. Confuse objects of similar shape. Have pt identify a number of objects of similar size and shape. Rule out Visual Agnosia.

Inability to perceive relationship of one object in space to another or to oneself. Have pt draw a clock.

Inability to perceive and interpret spatial concepts such as “up, down, under, over, in, out, in front of, and behind.” Arrange two different objects and have patient describe their relationships with each other.

Difficulty understanding and remembering relationship of one location to another. Ask patient to draw a familiar route.

Inability to recognize colors - unable to ID or name them. Have pt try to ID colors.
