Causes of PUD
A
- Helicobacter pylori
- Alcohol and smoking
- NSAIDS use
- Genetic susceptibility
- Hypercalcemia and chronic steroid use
- Chronic liver ,chronic lung and chronic pancreatitis
- Crohns and duodenal lymphoma or adenocarcinoma

What is the clinical presentation and what distinct features are there between duodenal and gastric ulcers
A
- Epigastric pain, radiating to the back
- Nausea and vomiting
- Weight loss/ gain
Distinct features
- Duodenal ulcers are alleviated by food or antacids and hence weight gain
- Gastric ulcers are made worse by food and hence weight loss

What are complications of PUD
A
- Perforation
- Bleeding
- Stomach outlet obstruction
- Penetration ( duodenal penetrating the pancreas)
- Carcinoma, common in gastric

Which ulcers must be biopsied to exclude malignancy
A
Gastric ulcers

How can H.pylori be diagnosed
A
- Antral biopsies taken to exclude malignancy
- Serology
- Breath tests

Indications for excluding Zollinger Ellison syndrome by measuring gastrin levels
A
- Patients that need PUD surgery
- Pts that are treated medical with following associations : diarrhoea, recurrent ulcers, multiple ulcers,atypical situated ulcers ,PU refractory to tx , hypertrophic gastric mucosa on endoscopy

Indications for PUD surgery
A
- Non- healing ulcers
- Complications : rare ones being bile duct strictures with obstructive jaundice and Fistulation into adjacent organs I.e gastro-colic fistula
