awake, cooperative, muscle relaxants reversed, intact gag reflex, vital capacity >10ml/kg, TV >6ml/kg, NIF >20cmH2O, SpO2 >90% on FiO2 40-50% with <5cm H2O PEEP, RSBI <100breaths/min/L

hypoxia differential
A
disconnected or inadequate O2 source, hypoventilation (inadequate TV or rate), atelectasis, extubation, mainstem intubation, PTX (2/2 iatrogenic), pulmonary edema (volume status, CHF, aspiration), aspiration (bronchospasm, PE, shunting, diffusion defect), PE, hemothorax, bronchospasm, decreased cardiac output (arrhythmia, HF, residual air in R heart), monitor malfunction
