weighs less than 24 pounds (11 Kg) or measures 33 inches or less.
Airway 2 points
Assisted airway with suctioning, adjuncts or Intubation
Consciousness 1 point
Amnesia, or a reliable Loss of consciousness
Consciousness 2 points
Altered mental status, or coma, Paralysis, or suspected Spinal Cord injury (decreased strength or sensation), Any loss of sensation.
Circulation 1 point
The carotid or femoral is palpable but neither the radial or pedal pulses are palpable. SBP <90 mm Hg.
Circulation 2 points
Weak or nonpalpable carotid or femoral pulses OR SBP < 50 mm Hg
Fracture 1 point
Suspected single CLOSED long bone fracture
Fracture 2 point
Any open long bone fracture, OR multiple fracture/dislocation sites (proximal to wrist or ankle).
Cutaneous 2 points
Major flap avulsions, soft tissue disruption, degloving injuries, etc. Amputation proximal to wrist or ankle; 2nd or 3rd degree burn to 10% or more BSA OR any high voltage electrical injury/lightning strike; OR penetrating injuries to head, neck or torso.
