Hydromorphone dosing (mg/kg)
0.015 mg/kg
Flumazenil (mg/kg)
0.01 mg/kg IV (max 0.2 mg/dose)
Neostigmine (mg/kg)
0.05-0.08 mg/kg
Naloxone (mcg/kg)
1 mcg/kg IV, titrate to Respiratory Rate
Atropine (mg/kg) for bradycardia
0.02 mg/kg IV or IM, higher doses may be needed for organophosphate toxicity. T1/2; 2-3hours(7hours if <2 years)
Epinephrine for Cardiac Arrest (mg/kg)
IV 0.01 mg/kg (0.1 mL/kg of 1:10,000) or ETT 0.1 mg/kg (0.1mL/kg of 1:1,000)
CaCL for Cardiac Arrest (mg/kg)
20 mg/kg IV, if not cardiac arrest infuse over 30mins
Calcium Gluconate dosing (mg/kg)
100 mg/kg IV
Dextrose for Hypoglycemia
0.5-1 g/kg (D50W 1-2ml/kg; D25W 2-4 ml/kg; D10W 5-10 ml/kg) note: D10W is 0.1g/mL
Joules for cardioversion in V tach (Joules/kg)
2 J/kg first shock; 4 J/kg 2nd shock
Medications that can be given via ETT
LANE; Lidocaine, Atropine, Naloxone, Epinephrine (give 2-3 times IV dose)
Adenosine dosing (mg/kg)
0.1mg/kg first dose, 0.2mg/kg 2nd dose
