PO Acetaminophen for infants and children
10-15mg/kg q4-6h prn
PO Acetaminophen for children 12 or older
325-1,000mg q4-6h
IV Acetaminophen dose for 2-12yo
15mg/kg q6h
Oral dose for n-Acetylcysteine
140mg/kg loading dose, then 70mg/kg q4h for 72 hours
IV dose for n-Acetylcysteine
300mg/kg in three doses over 21 hours
Activated charcoal dose for infants

Amiodarone dose for refractory VF, pulseless VT
- 5mg/kg rapid IV/IO
- Can repeat up to total dose of 15mg/kg in 24 hours.
- Max single dose 300mg
Amiodarone dose for SVT and ventricular dysrythmias
- Loading dose 5mg/kg IV/IO over 20-60 min
- Max single dose 300mg
- Can repeat up to a total dose of 15mg/kg in 24 hours
Atropine dose for bradydysrhythmias
- 0.02mg/kg IV/IO
- Min dose 0.1mg
- Max dose 0.5mg
- May repeat once
- Max total dose for <12 is 1mg
- Max total dose for 13+ is 3mg
Atropine dose for anticholinesterase poisoning
- <12: 0.05mg/kg/dose IV/IO
- May repeat at double first dose q20-30m until sx resolve
- > 12: 1mg/kg/dose IV/IO
- May repeat at double first dose q5m until sx resolve
Calcium Chloride dose
20mg/kg IV, may repeat prn
Dexamethasone dose
Single dose of 0.6mg/kg IV/IO, max 16mg
Diazepam dose for seizures
- 30d to 5yo: 0.2-0.5mg slow IV q2-5m, max 5mg
- > 5: 1mg q2-5m, max 10mg
Diphenhydramine dose
1mg/kg IM/IV/PO, max 25mg
Dopamine dose
2-20mcg/kg/min IV/IO, titrated to response
