Patient has right to refuse medication, RN responsibility is to educate why the medication is needed and inform prescriber if refused. Document as refusal

Assess needed labs, vital signs before giving medications

Verify MAR to order and hospital protocol

make sure patient has therapeutic effect of medication(s) without significant side effects
