Takes or verbalizes BSI precautions
2 - Scene Size Up
- Determines the scene/situation is safe
- Determines the MOI/NOI
- Determines the # of pts
- Requests additional help if necessary
- Considers stabilization of the spine
3 - Primary Survey (Part one)
- Verbalizes general impression of pt
- Determines responsiveness/LOC
- Determines CC/apparent life-threats
4 - Primary Survey (Part two)
Airway
- Opens and assesses airway
- Inserts an adjunct as indicated
5 - Primary Survey (Part three)
Breathing
- Assesses breathing
- Assures adequate ventilation
- Initiates appropriate oxygen therapy
- Manages any injury which may compromise breathing/ventilation
6 - Primary Survey (Part four)
Assesses circulation
- Checks pulse
- Assesses skin (either skin color, temperature, or condition)
- Assesses for and controls major bleeding if present
- Initiates shock management
7 - Primary Survey (Part five)
Identifies priority pts/makes transport decision based upon calculated GCS
8 - History Taking
- Obtains, or directs assistant to obtain, baseline VS
* Attempts to obtain SAMPLE history

Head
- Inspects mouth, nose, and assesses facial area
- Inspects and palpates scalp and ears
- Assesses eyes for PERRL

Neck
- Checks position of trachea

- Palpates c-spine

Chest
- Inspects chest
- Palpates chest
- Auscultates chest
12 - Secondary Assessment (Part four)
Abdomen/pelvis
- Inspects and palpates abdomen
- Assesses pelvis
- Verbalizes assessment of genitalia/perineum as needed
13 - Secondary Assessment (Part five)
- Lower extremities
- Inspects, palpates, and assesses motor, sensory, and distal circulatory functions
- Upper extremities
- Inspects, palpates, and assesses motor, sensory, and distal circulatory functions
14 - Secondary Assessment (Part six)
Posterior thorax, lumbar, and buttocks
- Inspects and palpates posterior thorax
- Inspects and palpates lumbar and buttocks area
15 - Secondary Assessment (Part seven)
- Manages secondary injures and wounds appropriately

Critical Criteria 1-3
- Failure to initiate or call for transport of the patient within 10 minute time limit
- Failure to take or verbalize body substance isolation precautions
- Failure to determine scene safety
Critical Criteria 4-6
- Failure to assess for and provide spinal protection when indicated
- Failure to voice and ultimately provide high concentration of oxygen
- Failure to assess/provide adequate ventilation
Critical Criteria 7-9
- Failure to find or appropriately manage problems associated with airway, breathing, hemorrhage or shock [hypoperfusion]
- Failure to differentiate patient’s need for immediate transportation versus continued assessment and treatment at the scene
- Does other detailed history or physical examination before assessing/treating threats to airway, breathing, and circulation
Critical Criteria 10-12
- Failure to manage the pt as a competent EMT
- Exhibits unacceptable affect with the pt or other personnel
- Uses or orders a dangerous or inappropriate intervention
