Determined when assessing the patient. Four elements are needed. Person Place Time Event Itself

Detailed physical Exam
Detailed exam of the different body systems with a patients which the complainant hasn’t been identified precisely
Focused history and physical examination
the part of the assessment process which patients chief complaints or any problems are immediately evident and then further and more specifically evaluated
Focused physical exam
Meant to identify a specific area if the body only done on responsive patients. it is driven by information gathered in the initial assessment and history exam
Glasgow Coma Scale (GCS)
Evaluation tool used to determine LOC which evaluates and assigns point of views (scores)
Eye opening

motor response
