Polymyositis
A
Symmetric proximal muscle weakness.
Endomysial inflammation with CD8+ T cells.
Commonly involves shoulders.
^ CK, +ANA, +Anti-Jo-1, +anti-SRP, +anti-Mi-2 antibodies.
Rx: Steroids followed by methotrexate

Dermatomyositis
A
Same at polymyositis +….
Malar rash, gottron papules (red flat topped papules with a light scale over bony prominences, often hands), “shawl and face” rash.
Increased risk of occult malignancy.
Perimysial inflammation and atrophy with CD4+ T cells.
