What is the clinical course of diabetic nephropathy?
Onset of diabetes to overt diabetic nephropathy (proteinuria >.5/24 hrs) usually 20 yrs or more
ESRD in another 5-10
What happens in extra capillary epithelial cell proliferation (crescent)?
RBC leaking and decrease filtration
Hematuria and acute renal failure
RBCs in urine, increase serum creatinine and BUN
How can you differentiate between wegeners, microscopic poly angiitis, and churg Strauss that all have lung and kidney involvement?
Wg - no asthma, granulomas
Mp - no asthma, no granulomas
Cs - asthma, no granulomas
What does an inflammatory infiltrate do in glomerular diseases?
RBC leaking and decreased filtration
Nephritic syndrome
RBCs in urine, increased serum creatinine and BUN
Examples are post infectious, proliferative, MPGN
What are the five types of renal injury seen with multiple myeloma?
Myeloma cast nephropathy - most common Amyloidosis Light chain deposition disease Heavy chain deposits disease Myeloma cell infiltrates in kidney - least common
What characterizes myeloma cell infiltrates in the kidney?
Monoclonal plasma cells in kidney parenchyma
