How is the passive voice formed?
A
Subject + form of Ser + past participle + por + agent (of action)
Example:
El hotel fue dañado por el tornado.
The hotel was damaged by the tornado.

Is the agent always explicitly stated in the passive voice?
A
No, often it is implied.
Example:
Fui despedido de mi trabajo.
I was fired from my job. (by someone known, or not important to mention.)

Is por always used in passive voice?
A
No. Very occasionally, speakers will use de. It does not change meaning.
Example:
La científica era admirada de todos.
The scientist was admired by all.
