Vitals
verbalize temp, RR, BP, in both arms @ level of heart
General appearance
Pt appears to be well dressed, appropriately groomed, with a calm mood and positive mental status
Skin, Hair, & Nails
no skin lesion or moles noted ; hair is consistently dispersed; scalp feels symmetrical with no bumps or lesions present ; nails have no sign of clubbing
eyes
i will now inspect the brows, lids, and lashes
the conjunctiva has no pallor
the sclera has no jaundice
the iris and pupils are an appropriate shape and size
snellen
this tests visual acuity at 20 feet.
“Your vision is 20/20 via the Snellen chart. A person with 20/20 vision can see what an average person can see on an eye chart when they are standing 20 feet away”
pupil exam
-peripheral vision test
-cover/uncover test for misalignment
-extraocular movements
-accommodation
-convergence
fundoscopic exam
assess for:
-red reflex visible
-optic disk + physiologic cup
-vessels leading from the disk have no sign of AV nicking or cotton wool patches
-retina is a deep red color
-macula has no vessels around it
ears inspection
the auricle, pinna, and tragus appear intact
-no pain to the pinna
otoscope exam
-external auditory canal free from obstruction
-tympanic membrane pearlescent w/o bulging, retraction, or presence of exudate
-cone of light & bony landmarks visible
hearing tests
-whisper
-weber: @ midline
-Rinne: @ mastoid bone
nose
-inspection and palpation of frontal & maxillary sinuses
-nasal patency
-turbinates, septum, and mucosa are visualized
mouth
inspection of lips, gums, teeth, buccal mucosa, tongue
-say ah: movement of soft palate
-pharynx tonsils, hard palette. uvula
-cranial nerve 12: tongue strength
neck
-tracheal placement midline
-palpation of thyroid from behind
-palpate lymph nodes
-auscultate carotid for bruit
chest & thorax
-resp effort and depth are unlabored with no use of accessory muscles
-inspection of chest shows normal shape & size, symmetrical movement, no retractions
-skin is free of scars, bruises, and lesions
-auscultation
heart
-palpation for thrills or pulsations
-auscultation in all 5 spots both sitting and squatting
-JVD on R side - JVD pressure usually 3-8 cm h20
