What is hypospadias/epispadias?
- congenital defect
- urethral fold fails to fuse completely over urethral groove
urethral meatus on ventral surface of penis
hypospadias
urethral meatus on dorsal surface of penis
epispadias
When can a child with hypospadias be circumcised?
- once the opening is corrected (foreskin may be used for surgical repair)
- repair before potty training, but best within 1st year of life
When might hypospadias occur and what defects might be associated?
- may occur with congenital chordee (shortage of penis skin causing the penis to bow)
- associated defects: undescended testes
- partial absence of foreskin
What do we need to do for post-op care after hypospadias?
- care of surgical site: color and temp of penis
- care for urethral stent: double diaper
- pain management: antispasmodics and analgesics
- known as a nephroblastoma
- malignant renal and intra-abdominal tumor of childhood
- Wilms Tumor
- more common in AA children and males
What is the peak age of diagnosis of a Wilms Tumor?
3 years
What are the clinical manifestations of a Wilms tumor?
- painless
- swelling or mass within the abdomen
- abd firm and contender
- usually discovered by parent
What should you never do during the physical assessment of a child with a Wilms Tumor?
Do not palpate the abdomen
What is the survival rate for a child with a localized Wilms tumor?
90% with multimodal therapy
