an unexpected occurrence involving death or serious physical or psychological injury or the risk thereof.
Types of Sentinel Event common in maternal child
Leaving a foreign body in a client (sponge, 4x4, forceps)
Falls
Maternal death related to the birth process
Wrong milk to wrong baby
Birth injury
GTPALM
Gravida, Term deliveries, Preterm deliveries, Abortions, Living children, Multiples
What is a term pregnancy?
37 weeks to 42 weeks
What is a preterm pregnancy?
20 weeks to 36.6 weeks
What is consider abortion?
Less than 20 weeks
What are all pregnant women screened for? Who is re-screened?
HIV, Hepatitis B virus, Syphilis, Chlamydia, and Gonorrhea 1st trimester, Hepatitis C)
Women at risk (multiple sex partners; drug abuse) rescreened 3rd trimester
Lab test/ results for chlamydia
When: Screen 1st trimester or when enter healthcare
How: culture or DNA probe, or enzyme immunoassay. CDC recommends nucleic acid amplification test (NAAT) or urinary, vaginal or endocervical areas
Retest 3rd trimester/ if multiple sex partners or younger than 25
Lab test/ results for Gonorrhea
Gram stain culture of endocervical, vaginal, rectum and possibly pharynx
Also, chlamydia culture and serologic test for syphilis
Screened at 1st prenatal visit. At risk clients screened again in 3rd trimester (36 weeks)
Lab test/ results for Syphilis
Screened at first prenatal visit VDRL or RPR serology and again in 3rd trimester and at time of birth if they are high risk.
(If HIV or other STI always check to see that a RPR or VDRL was done too)
Toxoplasmosis
(Protozoan) infection effects:
Mom: Influenza-like aching Lymphadenopathy Spontaneous abortion
Baby: Congenital toxoplasmosis, LBW; hepatosplenomegaly c Jaundice and anemia.
Varicella Zoster (chicken pox) infection effects:
Mom: Preterm labor, encephalitis, and varicella pneumonia.
Baby: Congenital varicella syndrome c limb hypoplasia, cataracts, microcephaly, and symetric IUGR during 1st trimester.
Rubella infection effects:
Mom: Rash, fever , malaise. Spontanteous ab during 1st trimester of pregnancy.
Baby: Deafness, MR, IUGR, cardiac defects and microcephaly
Cytomegalovirus (CMV) infection effects:
Mom: Flu-like symptoms; cervical discharge
Baby: Fetal or neonatal death; Severe generalized disease c hemolytic anemia, jaundice, hydrocephaly or microcephaly.
Hepatitis B infection effects:
Mom: Fever, rash, arthralgia, abdominal pain, liver enlarged and tender.
Baby: Prematurity; LBW. Development of acute infection at birth and perhaps neonatal death.
