What is a marker of advanced systemic atherosclerosis?
PAD
how do the arteries get narrowed?
deposits of cholesterol and lipids within the vessel
-exact cause is unknown but inflammation and endothelial injury play a bog part
where is the problem if you have caludication in the butt or thighs?
iliac arteries
Where is the problem if you have caludication in the calf?
femoral or popliteal
What is a classic sign of PAD?
caludication caused by exercise
-resolves within 10 monutes
Symptoms of PAD critical limb ischemia?
rest pain lasting more than 2 weeks
arterial leg ulcers
gangrene
What meds are used to treat PAD?
ACE inhibitors
STATINS
oral antiplatelet therapy: ASA, clopidogrel (Plavix)
Intermittent caludication: cilostazol and pentoxifylline
What do we need to teach pt’s about with taking plavix?
omeprazole (prilosec) reduces the effects of plavix by half
What are the IR procedures for CLI from PAD?
PTA: percutaneous transluminal angioplasty
stent
atherectomy
cryoplasty (PTA and cold therapy)
- nonatherosclerotic recurrent inflammatory disorder
- can be confused with PAD or other autoimmune
- intermittent claudication of feet, hands, arms
- chronic periodontal infection

What is the tx for buergers disease?
complete cessation of tobacco and marijuana use
