What is Parkinson’s?
There is progressive reduction in dopamine in the basal ganglia => disorder of movement
What is the typical presentation?
Asymmetrical
Old man, around 70
Gradual onset symptoms
What is the classic triad in Parkinson’s?
Resting tremor
Rigidity
Bradykinesia
What are other features of Parkinson’s?
Depression
Sleep disturbance and insomnia
Loss of the sense of smell (anosmia)
Postural instability (increasing the risk of falls)
Cognitive impairment and memory problems
What dementia is associated with Parkinsonism features?
Dementia with levy bodies
How is Parkinson’s diagnosed?
Clinically based on history and examination findings
What are treatemnt options?
Levodopa (combined with peripheral decarboxylase inhibitors)
COMT inhibitors
Dopamine agonists
Monoamine oxidase-B inhibitors
What is levodopa?
Synthetic dopamine
What is the main side effects of levodopa?
Dyskinesia
What can manage the dyskinesia associated with levodopa?
Amatadine a glutamate antagonist
What do dopamine agonists do?
Mimic the action of dopamine in the basal ganglia
When are dopamine agonists used?
Used to delay the use of levodopa
What is a side effect of dopamine agonists?
Pulmonary fibrosis
What are examples of dopamine agonists?
Bromocriptine
Pergolide
Cabergoline
