Paraphilic Disorders
characterized by an intense and persistent sexual interest other than sexual interest in genital stimulation or preparatory fondling with phenotypically normal, physically mature, consenting human partners that is currently causing distress or impairment to the individual or has entailed personal harm or risk of harm to others
Frotteuristic Disorder
characterized by intense sexual arousal from touching or rubbing against a nonconsenting adult as manifested by fantasies, urges, and/or behaviors and a history of acting on the urges with a nonconsenting person or experiencing significant distress or impaired functioning as the result of fantasies and urges
usually begins in adolescence and behaviors associated with it often decline with increasing age
Transvestic Disorder
involves cross-dressing for the purpose of sexual arousal as manifested by fantasies, urges, and/or behaviors that cause significant distress or impaired functioning
diagnosis has been reported almost exclusively in males and the majority of males identify as heterosexuals, although some individuals have occasional sexual interaction with other males, especially when they are cross distressed
Treatment of Paraphilias
in past, most common treatment was in-vivo aversion therapy; however, this technique only has short-term benefits
current treatments ordinarily utilize a CBT approach that combines cognitive statements and tolerate strong emotions that may precipitate inappropriate sexual behaviors and behavioral strategies that target sexual fantasies, urges, and behaviors such as :
Covert sensitization- aversive conditioning in imagination
Orgasmic reconditioning- involves having the individual replace the unacceptable sexual fantasy with a more acceptable one while masturbating
Drug medroxyprogesterone (DepoProveral reduces paraphiliac behaviors in many men, a drawback is that its benefits stop as soon as it is discontinued
