Clinical features
A
- intermittent dysphagia
- chest pain due to distension
- postprandial SOB
- postprandial vomiting
- palpitation due to pressure on heart
- epigastric pain
- anaemia secondary to ulceration (Cameron ulcer)

Investigation
A
- CXR: air/fluid level in mediastinum
- OGD
- Barium swallow
- CT scan: delineate extent of hernia

Complication
A
Acute gastric volvulus (stomach rotate >180–> closed loop obstruction–> strangulation and necrosis)
- Borchardt’s triad: severe epigastric pain, unproductive retching, inability to pass a nasogastric tube

Treatment
A
- hiatal repair combined with either fundoplication or gastropexy
- large hiatal defects: reinforcement with biological mesh
