Secretin
released from the duodenal mucosa in response to the presence of acid in the duodenum.
stimulates the release of bicarbonate and water.
Cholecystokinin (CCK)
released from SI endocrine cells in response to the entry of fats and proteins into the proximal intestine.
acts directly and through vagal afferents to stimulate pancreatic acinar cells to release digestive proenzymes.
Pancreatitits Lab Tests
Serum amylase:
Serum lipase: (#1 test for pancreatitis)
sensitivity 82-100%, more specific than amylase
CRP: to monitor disease, not diagnose
Virchow’s node
L supraclavicular node, seen in pancreatic cancer
Tumors of the head present w/
weight loss, steatorrhea & jaundice
Tumors of the body or tail present w/
pain & weight loss
Serum tumor maker
CA 19-9 closely related to tumor size, can be elevated in pts w/ benign pancreaticobiliary disorders, serial monitoring can assess the response to treatment
