What is advanced Care Planning?
Involves discussing and making decisions about future medical care if one becomes seriously ill.

- who will care for you

Documenting ACP
Document your decisions:
- advance healthcare directives
- Name a healthcare proxy
- DNR order
-Not legally binding, but the healthcare practitioner can accommodate some of the aspects stated
- Can be changed at any point.
