What is step 1 of the analegesic ladder?
Non-opioids (e.g. paracetamol or aspirin)
What is step 2 of the analgesic ladder?
Weak opioids (e.g. codeine)
What is step 3 of the analgesic ladder?
Strong opioids (e.g. morphine)
How is ‘background’ pain treated?
Slow-release medication.
How is ‘breakthrough’ pain treated?
Immediate-release medication.
What can be given if pain does not respond to opioids?
Addition of an adjuvant or alternative drug therapies (e.g. gabapentin).
How does opioid toxicity present?
Hallucinations
Myoclonus
Drowsiness
Respiratory depression (if life-threatening)
What is myoclonus?
Involuntary twitching of the limbs.
How can respiratory depression following opioid overdose be reversed?
Naloxone
What is the first-line strong opioid?
Morphine
What is the second-line strong opioid?
Oxycodone
What strong opioid is used in those with severe renal impairment?
Fentanyl
What treatable conditions can mimic dying?
Opioid/Drug toxicity Sepsis/Infection Hypercalcaemia Acute kidney injury Hypocalcaemia
What can be used when oral medications become too difficult to take?
Syringe drivers
What is a syringe driver?
A pump, allowing continuous subcutaneous infusion of drugs.
Can give 3 together within this.
