Paget's Disease Flashcards
What is Paget’s disease?
Chronic disease that results in thickened, brittle and mis-shapen bones
Who is affected by Paget’s disease?
Common
- Age related
- Affects elderly
- Rarely affects young
What causes Paget’s disease?
Age related
Genetic Cause
Describe the pathophysiology of Paget’s disease?
Increased bone reabsorption (increased osteoclast activity)
Compensation by increased bone production (increased osteoblast activity)
New bone is produced but it is brittle with thick cortices
What bones are affected by Paget’s?
Femur
Tibia
Pelvis
Skull
Ear Ossicles
Describe the symptoms of Paget’s?
Can be asymptomatic
Can cause arthritis
Can cause pathological fractures
Deformity
Pain
Describe cardiac symptoms of Paget’s?
High output cardiac failure
Due to increased blood flow through Pagetic bone
Describe ear symptoms of Pagets?
Can affect ear ossicles
Leads to conductive deafness
Describe the biochemistry of Paget’s disease?
Normal Calcium
Normal Phosphate
Normal PTH
Increased ALP
Describe what happens to ALP when there is active bone formation?
ALP rises when there is active bone formation
Describe the radiological findings of Pagets?
Thickened cortices
Coarse bones
Areas of lysis and sclerosis
Describe the results of a bone scan?
Increased uptake
Treatment of Paget’s?
Biphosphonates
Calcitonin
Joint replacement
Fracture Stabilization
What does this photo show?

Areas of lysis and sclerosis
Deformity
Give the biochemical results for osteoporosis?
Calcium Normal
Phosphate Normal
ALP Normal
PTH Normal
