Differences between bacterial and viral pneumonia
Bacterial
- age >2
- absence of wheeze
- absence of rhinorrhoea
- temp >38
- localised pain
Hospitalisation of pneumonia criteria
O2 sats <92
Reduced feeds
Treatment or pneumonia Paeds
Oral amoxicillin, coamxiclav if unresponsive
In neonates, IV amoxicillin
May give IV if more severe
If no resolution >48hrs of Abx = parapneumonic effusion needing drainage
Follow up with CXR in 4 weeks if atelectasis
Croup treatment
Hydration, nutrition and apyrexial Mx
Admit if severe (>8 on westley score)
Single dose PO steroids
NEB adrenaline
Features of bronchiolitis
High pitched paroxysmal cough Wheeze Fever Signs of resp distress, may get apneas Recessions Reduced feeds
Treatment of bronchiolitis
Maintain feeds, fluids and apyrexial measures
Can give O2 if required
Admission criteria for bronchiolitis
Risk factors for OSA
Craniofacial abnormalities- Down’s
Muscular dystrophy - Duchenne’s
Adenotonsillar hypertrophy
Severe obesity
Presentation of OSA
- daytime tiredness
- snoring
- learning and behavioural problems
Treatment of OSA
- risk reduction: weight loss
- CPAP or NIV
- adenotonsillectomy
