Fatigue, nausea, weight loss, orthostatic hypotension, and hyponatremia in a patient with low serum cortisol suggest adrenal insufficiency. The failure of cortisol to increase after ACTH stimulation test confirms the diagnosis. Certain clinical manifestations such as hyperpigmentation and hyperkalemia are only seen with primary adrenal insufficiency (Addison disease).
Cosyntropin stimulation test, also known as ACTH stimulation test
for suspected primary adrenal insufficiency. Serum cortisol levels are measured before and after the patient is given exogenous ACTH. In a patient with normal adrenal function, the cortisol levels should rise dramatically in response to the ACTH, whereas in a patient with primary adrenal insufficiency, cortisol levels should remain low.
