Acetaminophen (APAP)
Analgesic / Antipyretic
* Adult: 650 mg Orally (Q4hr PRN)
* Pediatric: Follow Weight Based Dosing Chart
*Usually 15 mg/kg Orally (Q4hr PRN)
Maximum 4 grams in 24 Hours*
Activated Charcoal
Chemical Absorbant
* 1 gm/kg Orally (75gm is Typical)
Adenosine (Adenocard)
Class-V Antidysrhythmic
* Adult: 6 mg (Rapid IV/IO) followed by a Rapid Saline Flush 20 mL
*2 Repeat Doses of 12 mg (IV/IO) may be given.
- Pediatric: 0.1 mg/kg (Rapid IV/IO) followed by a Rapid Saline Flush 5 mL
*2 Repeat Doses of 0.2 mg/kg (IV/IO) may be given.
Albuterol (Proventil/Ventolin)
Bronchodilator / Beta II Agonist
* Adults/Peds: 3 mg Albuterol / 0.5 mg Ipratropium diluted in 2.5 mL of Normal Saline
Nebulized over 5-15 Minutes
Amiodarone
Class–III Antidysrhythmic
Ventricular Fibrillation and Pulseless Ventricular Tachycardia:
* Adult: 300 mg (IV/IO)
* Pediatric: 5 mg/kg (IV/IO)
Wide Complex Tachycardias (With Pulse):
* Adult: 150 mg (IV/IO) – Infused over 10 Minutes
* Pediatric: 5 mg/kg (IV/IO) – Infused over 20-60 Minutes
Aspirin (ASA)
Antiplatelet; Non-Opioid Analgesic; Antipyretic
* Adult: 325mg PO (Often dosed as four 81mg tablets)
Atropine
Parasympatholytic (Anticholinergic/Antimuscarinic)
Symptomatic Bradycardia:
* Adult: 1mg (IV/IO) every 3 – 5 Minutes with a Max Dose of 3 mg.
* Pediatric: 0.02 mg/kg (IV/IO) May Repeat Once
Nerve Agent Poisoning:
* Adult: 2 to 6 mg (IV/IM/IO)
* Commonly Administered via “Rescue Pens”: Nerve Agent Antidote Auto-Injector (NAAA) or Atropen Injector
Calcium Chloride
Electrolyte (Anion)
* 1 Gram of a 10% Solution (IV/IO) – Slowly
- Do not administer by IM or SubQ routes; it will cause significant tissue necrosis.
- IV should be FLUSHED between administrations with Sodium Bicarbonate to avoid precipitation.
Cefazolin (Ancef)
Antibiotic (Broad Spectrum)
* Adults: 1-2 g (IV/IO Infusion)
* Children Above 9 Years old: 1 g (IV/IO Infusion)
Diazepam (Valium)
Benzodiazepine
* Adult: 5-10 mg (IV/IO)
o May Repeat After 5 Minutes
* Pediatrics: 0.3 mg/kg (IV/IO)
o May Repeat After 5 Minutes
Diltiazem (Cardizem)
Calcium Channel Blocker
* Adult: 0.25 mg/kg (IV/IO)
o Slow IV Push
* Use extra caution in patients who take Beta Blockers, as combining these two drugs will potentiate each other’s effects and toxicities.
* Patients with a history of heart failure and heart block are at high risk for toxicity.
Diphenhydramine (Benadryl)
Antihistamine
* Adult: 25-50 mg (PO/IV/IM/IO)
* Pediatrics: 1 mg/kg (PO/IV/IM/IO)
Dopamine
Sympathomimetic; Vasopressor
* Adult: 2-20 mcg/kg/min (IV/IO) Infusion.
* Pediatric: Dosing Same as Adult
- Dose Titration to Desired Effect
- Typical Starting dose is 5 mcg/kg/min.
Droperidol (Inapsine)
Sedative-Hypnotic; Antiemetic
* Antiemetic Dose: 1.25 mg (IV/IM/IO)
* Sedation Dose: 5mg (IM)
Enalapril (Vasotec)
ACE Inhibitor
* 0.625 mg – 1.25 mg IV
Epinephrine (Adrenalin)
Sympathomimetic; Adrenergic Agent; Inotropic; Chronotropic
Cardiac Arrest
* Adult: 1mg (1:10,000) IV/IO
* Pediatric: 0.01 mg/kg (1:10,000) IV/IO
Anaphylaxis/Respiratory
* Adult: 0.3mg IM (1:1,000)
* Pediatric: 0.15mg IM (1:1,000)
