NEVER accept money but small gifts ok
(importance) patients wishes
very important?
when are you allowed to breach confidentiality
- when patient has given consent
- if there is a potential risk to society (a doctor is obliged to infrom the DVLA if a patient has seizures and has not reported them as there is a risk when driving)
PATIENTS HAVE A RIGHT TO BE INFORMED OF ANY BREACHES OF CONFIDENTIALITY
if you do need to share information, only the required info should be given and in a private setting
what are the top priorities for confidentiality
- stop the breach as soon as possible
- do not draw attention to the event (inappropriate but not awful)
- prevent any future breaches

in a scenario when it is unclear whether a person did or did not cheat/do something
you would still need to pursue the situation and find out more information
appropriate - report possible cheating to the medical school
appropriate
CHEATING
- the main factor of importance is the necessity for the problem to be reported
most other factors are not so important as the issue should be raised regardless
importance - whether or not an event happened a long time ago (cheating)
not important at all - should still be reported
appropriate - to offer medical advice ,perform unsupervised procedures as a medical student
very inappropriate: must aslways be honest in your skills and work within your limits

very appropriate
CONTEXT IS CRUCIAL - a patient may look confused but not actually be so

very important
THE DOCTOR IS RESPONSIBLE FOR THEIR SHADOWING STUDENTS’ WORK

important
CONSIDER URGENCY WHEN YOU OBSERVE SOMEONE MAKING A MISTAKE
if its a medical student ina harmless procedure then you can wait and inform them what they did wrong in private, if doctor and lifethreatening act immediately
appropriate - find out what caused the mistake
appropriate
appropriate - report a mistake if it did not cause harm/was not spotted

very inappropriate - they should deal with the situation themself
appropriate - reporting a serious issue without informing the patient of a breach of confidentiality
appropriate but not ideal - reporting is the right move but ideally the patient would be told
CONSENT MAY NEED TO BE GIVEN BY PARENTS FOR CHILDREN UNDER 16 YEARS
SELF PLAGIARISM = REUSING YOUR OWN WORK THAT HAS ALREADY BEEN MARKED
CONSULTANT DOESNT NEED TO KNOW ABOUT A POTENTIAL BREECH OF CONFIDENTIALITY
importance - fiona is about to perform a procedure as part of an assessment when she gets a call about her grandma being sick.
the welfare of fionas grandmother
important as it might impact on her ability to focus on the task
consult more senior doctors on matters of confidentiality as seomtimes it is much more complicated due capacity of a patient so wihtholding information from a patient is a complex decision
importance - considering a supervisor’s instructions
very important
ON ANY OCCASION THAT A PATIENT IS WORRIED ABOUT A PROCEDURE, THEY SHOULD BE REASSURED BY ANYONE INVOLVED IN THEIR CARE
approproiate - telling a tutor that she must be allowed to switch tutor groups
very inapproripate - should never TELL a tutor what they must and must not do
importance - a parent of a young child’s wishes
important but not very important
importance - a patient/patient’s parent’s right to make decisions about patient care
very important
appropriate - suggesting rescheduling an appointment before any attempt is made to calm the patient down
appropriate but not ideal
SHOULD PUT PATIENTS INTERESTS BEFORE OWN so do not push/persuade too hard to do something even if it would mean a missed opportunity
IF SOMEONE IS SAID TO BE UNRELIABLE
- in the case that a patient is involved it is of minor importance, assume what they are saying is true and treat the patient immediately
- in the case of rumours/cheating, it is important as you will need to find out the truth before acting
if someone is being agressive - whether or not there is someone else nearby to help
very important
if a doctor does anything to compromise patient safety, breach of patient confidenitality or data handling - ignoring and monitoring would be very inappropriate, otherwise it can be inappropriate but not awful
UNLESS THERE IS A MEDICAL EMERGENCY IT IS RARELY APPROPRIATE TO CALL SOMEONE IN OFF A BREAK
