Primary orbital tumors - Symptoms
Exophtalmus, otherwise no symptoms. Diplopia and loss of red- green and total vision is late. NB: secondary tumors of orbit present with same signs and symptoms.
Primary orbital tumors - Types
- Glioma - very common in US, not Europa
- Hemangioma covernosum - most common in adult. Round and encapsulated on CT
- Melanoma
- Osteoma
- Top 3 pediatrics: dermoid cysts, capillary hemangioma and rhabdomyosarcoma
Top 3 adult: lymphoid tumors, cavernous hemangiomas and meningiomas
Primary orbital tumors - Treatment
Lateral orbitotomy, extended canthotomy, also possible to enter via nasal way.
Graves’ exophtalmus
Due to hypertrophy of muscles in orbit.
Lacrimal gland malignancies - origin
Most are epithelial in origin.
Lacrimal gland malignancies - malignant/benign
50/50 benign and malignant. Most common malignant is adenoid cystic carcinoma. Peak incidence is 30s and teenage years.
Lacrimal gland malignancies - symptoms
asymptomatic with slight fullness of temporal upper lid to frank proptosis, diplopia and encroaching mass lesion
Lacrimal gland malignancies - Differential diagnosis
Inflammatory prossess, dry eye syndrome, adult ptosis
Lacrimal gland malignancies - Treatment
Radiation and/or radical excision
Lacrimal gland malignancies - Survival rate
15 year survival = 75%
