Definition: Bicipital Tendinopathy
Inflammation of biceps brachii long head tendon and surrounding sheath
DDX: Bicipital Tendinopathy
Usually co-exists with other pathologies of the shoulder including: rotator cuff tendinopathy & tears, GH instability and other muscle imbalances
Risk factors
Repetitive overhead activities, prior shoulder injuries
Sports specific risk factors
Pitchers, swimmers, gynasts, racquet sports, rowing
Palpation
Hallmark: local tenderness over the bicipital groove
Special tests
Speed test, hyper extension test, yergason
Yergusons test
Resisted supination while externally rotating. Subluxation at bicipital groove
Speeds test
CT arm straight out in supination. MT presses down. Pain in bicipital groove
Hyper-extension test
Passive hyper-extension of the arm to test the flexor muscles on stretch and anterior GH joint involvement
Definition: Subluxation
- slight misalignment 2. partial dislocation
General Rx
PRICE to reduce pain and inflammation
Massage Therapy
Acute: swedish, mfr Recovery: cross fibre friction over bicipital groove
Medication
NSAIDS for 2-3 weeks to reduce pain & inflammation OR steroidal injection into biceps sheath
Rehabilitation (circumduction)
ROM exercises: stretching biceps tendon, rotations, pendulum weights
Rehabilitation (exercises)
Lateral/front finger walking, isometrics progress to concentric exercises, then to eccentric exercises, lastly to sport-specific exercises
