Overdenture Abutments Flashcards
Define over dentures
A prosthesis that derives support from one or more abutment teeth (implant) by completely enclosing them beneath its impression session
How much resorption occurs of residual alveolar ridges following extraction
0.25-0.4mm annual bone loss following extraction
Which area of the mouth Is most affected by resorption of residual alveolar ridges following extraction
Anterior mandible
What are the advantages of over dentures
- Less resorption of alveolar bone
- Improved sensory feedback
- Improved rertention
- Decreased psychological trauma
What are some of the disadvantages of overdentues
- Expensive
- More complicated maintenance
- Weakening of denture base
- Excellent OH requirement
- Potential for caries and periodontal disease
Give some indications for over dentures
- Suitable teeth present
- Complete of partial denture required
- Sufficient inter occlusal space
- Cleft palate/ surgical defect where additional tooth borne support would be beneficial
- Severe toothwear with healthy perio dontium
Give some contra indications for over dentures
- High caries risk and patient not modifying behaviour
- Subgingival caries on abutment teeth
- Active periodontal disease and poor bone support
- Poor OH or patient not able to maintain high level of plaque control
- Any condition which precludes root canal treatment
What criteria must an abutment tooth fulfil to qualify for an over denture
- Need 6mm bone support+ good width of attached gingiva
- No sub gingival caries
- RCT possible to complete
- Teeth symmetrically distributed in the arch
- At least 2 abutments required
- Cleansable
Which teeth are the best over denture abutments
- Canines
- Premolars
- Molars
- Incisors
How can we modify a tooth for an over denture
- Simple dome following RCT and composite over GP
- Onlay/ coping
- Vital thimble crown
- Root face attachment
- Bars
- Magnets
