Rupture of cyst and contents into peritoneal cavity –> intense pain (especially if dermoid cyst or endometrioma)

Haemorrhage into a cyst or peritoneal cavity –> causes pain and can cause shock

Torsion of the pedicle –> infarction of the ovary + tube with severe pain
Urgent surgery required
