Features on history
Fever Ear pain Irritability \+/- anorexia Vomiting Lethargy Recent Hx of URTI
Features on examination
Dull + opaque tympanic membrane. May be bulging.
May be yellow- gray in colour
Middle ear landmarks (incus, handle of malleus, light reflex) are not well seen
+- febrile
Associated signs of URTI - cough coryza, red tonsillopharynx
Common complication often seen on AOM
- febrile convulsions common
- perforation- provides relief
Mx
Most cases resolve spontaneously
Pain is main Sx, adequate analgesia : paracetamol 20-30mg/kg for 2-3 doses a day
If >12m, mildly unwell, not immunocompromised- just analgesia and no ABs for 24-48h
If no improvement- analgesia + amoxicillin 15mg/ kg/ dose TDS for 5 days
Advise parents to seek medical RV of ear Sx, heading difficulty of persistent irritability after 2-3 months (for effusion)
