Arthritis
What: inflammation of joints
Symptoms: sensory impairments, pain, swelling, deformities, limited ROM, fatigue
Interventions: •joint protection principles •energy conservation principles •adapted equipment •PAMs- heat/paraffin •splints •strengthening
Upper Extremity Amputation
What: loss of any part of arm or hand
Symptoms: insensitivity, pain, skin integrity, postural changes
Effects: body image, hand dominance, compensation, bilateral capabilities
Interventions:
•adaptive equipment
•prosthetic training
Pre-prosthetic Treatment
o Change of dominance activities o ROM of uninvolved joints o Prep limb o Desensitization o Wrap to shape and shrink residual limb o ADL training
Prosthetic Treatment
o Functional training with prosthesis
o Practice engagement in activities
o Don and doff prosthetic
o Increase wearing tolerance
Types of Burns
o Superficial/1st degree o Superficial partial thickness o Deep partial thickness o Full Thickness o 4th degree
Superficial/1st degree burn
o epidermis only
o minimal pain/edema, no blisters
o healing time 3-7 days
Superficial Partial Thickness Burn
o 2nd degree burn o epidermis and upper portion of dermis o red, blistering, wet o painful but no grafting necessary o healing time 7-21 days
Intervention: •Wound care, debridement, sterile whirlpool, dressing changes • Gentle AROM and PROM • Edema control • splinting
Deep Partial Thickness Burn
o Deep 2nd degree burn
o Epidermis and deep portion of dermis including hair follicles and sweat glands
o Red, white, and elastic
o Sensation may be impaired
o Potential to convert to full thickness burn if infected
o Healing time: 21-35 days
Intervention: • Wound care, debridement, sterile whirlpool, dressing changes • Gentle AROM and PROM • Edema control • splinting • strengthening
Full Thickness Burn
o 3rd degree burn
o involves epidermis, dermis, hair follicles, sweat glands, nerve endings
o white, waxy, leathery, non-elastic
o Sensation is absent, requires skin graft
o Hypertrophic scar
o Healing time- months
Intervention:
- 72 hours post op: dressing changes, splint at all times
- 5-7 days: begin AROM, light ADL, meaningful activities, sterile whirlpool
- +7 days: PROM as tolerated, ADL
- massage when wounds heal
- compression garments
- strengthening
4th Degree Burn
o involves fat, muscle, bone
o Electrical Burn: destruction of nerve along pathway
Splints for burns
o Hands: • Wrist in 20-30° extension • MCP joints 50-70° flexion • IP fully extended • Thumb abducted and extended • Volar burns: Wrist in 0-30° extension MCP joints neutral to slight extension and abducted IP joints full extension Thumb abducted and extended
