heightened chronic pain response after injury cases a constant burning or throbbing pain and sensitivity to simulate which is not normally painful (i.e. cold or light touch), can be caused by a peripheral nerve injury (type 2) but this is rare and is usually idiopathic (type 1)

- if acute can be managed with antibiotics plus or minus a surgical washout
- if longer than a few weeks can be kept at bay with antibiotics but then metal component should be removed
- if infection causing sepsis or draining sinus then all metal components must be removed and bone derided immediately
